Form to Request Activity and Program Accommodations
Rowan County School District shall furnish appropriate auxiliary aids and services where necessary to afford individuals with disabilities/impairments, including applicants, participants, companions, and members of the public, an equal opportunity to participate in, and enjoy the benefits of, a service, program, or activity of the Rowan County School District.
Individuals that require an auxiliary aid or service should notify the school at least ten (10) days prior to the event during which time the auxiliary aid or service would be needed.
Request for Home/Hospital Instruction
Request for eligibility for home/hospital instruction for students with disabilities to be determined by the Admissions and Release Committee (ARC) in accordance with their Individual Education Program (IEP), with the services to be in the least restrictive environment.
Any child who is excused from school attendance more than six (6) months must have two (2) signed statements from two different local health personnel which can be a combination of the following professional persons: a licensed physician, advanced registered nurse practitioner, psychologist, psychiatrist, chiropractor and health officer. If a medical professional certifies that a student has a chronic physical condition unlikely to substantially improve within one (1) year, then the one signed statement is sufficient for services that extend beyond six (6) months. This exception does not apply to students with mental health conditions.

Site-Based Decision-Making Councils & Meeting Dates
Each school has its own Site-Based Decision-Making council whose membership includes parent representatives, teacher representatives and the school principal. These councils provide parents with the opportunity to work with the teachers and school administration to make decisions related to the day-to-day operations of their children’s schools. All meeting of the Site-Based Decision-Making councils are open to the public.

KHSAA Physical and Parental Permission Form (High School)
KHSAA Physical and Parental Permission Form (Middle School)
All students interested in participating in a school sport must have a sports physical. The physical can be completed by a physician, a physician assistant, or a nurse practitioner. The KHSAA Physical and Parental Permission Form must be signed by the physician, the parent/guardian, and the student and then returned to the school.
